Please complete the form below to create a return label for your sample(s). Please reuse the kit box and attach the return postage label created over the original postage label. Once the label is created, you can also arrange free collection of your samples via Royal Mail. Please ensure everyone is aware that the collection is taking place. Your test results will be ready in approx 14 days from the date of collection. We will email you when they are available.
